1. Health insurance

Dellent offers Health Insurance to all employees. There are two plans:

• Option 1, which is the plan assigned by Dellent;
• Option 2 and 3, which is an upgrade available, that you can access by paying an additional monthly fee.

2. Changes and/or additions to health insurance

Any interest in changing your health insurance, either adding family members or upgrading your plan, must be communicated to the person responsible for People & Culture.

Values:

• Plan 1: Adult - 241,11€/ year; Child up to 25 years old - 192,89€/year;
• Plan 2: Adult - 330,51€/year; Child up to 25 years old - 264,41€/year;
• Plan 3: Adult - 401,62€/year; Child up to 25 years old - 321,30€/ano;
• Upgrade Plan 2: 89,4€/year.
• Upgrade Plan 3: 160,51€/year.

Procedure for inclusion of a family member

Provide the following details of the family member and the plan you wish to benefit from

• Full name;
• NIF;
• Degree of kinship;
